Students have to fill this initial application form and submit it to the Student Exchange Program Director. Students applying for the Student Exchange Program must make sure they have read the eligibility requirements and selected a school from the MOU List.

Students applying for the Student Exchange Program must fill this form for approval of courses to be studied abroad by the Dean of Admission & Registration and the Dean of Student Affairs.

Students must only fill this form after the approval of the Student Exchange Program Director with the courses they are willing to study at that school. This form will be sent to the school abroad.
